“Insanity is doing the same thing over and over but expecting different outcomes.” Yes, indeed. Post-AICPA BV and Healthcare Conferences, my memories of the post-Clinton Era Reform (attempt) are clear.  As I said last week in my various Conference sessions, the nomenclature and acronyms have changed,* but the theme remains the same. The Primary Care Physician Gatekeeper Model is now a “Medical Home.” A capitated Integrated Delivery System is now an “Accountable Care Organization.” That said, a horse is not a cow and a rose by any other name would not smell like a pasture. After 100’s of hours of studying the legislation and watching the response of the provider, insurer and political communities thereto, I have as yet to find optimism.

*Brings to mind Jack Webb: “The names have been changed to protect the innocent.”
